Who fills it in
Whoever owns credentialing at the practice, one copy per provider. Check off each document as it lands in the file, then use the payer tracking table to record where every application stands, so nothing waits on a missing W-9 for three weeks.
Before you hand it out
The last page of the PDF is a note to the practice, with a reminder to remove it before the packet is copied or handed out. Review the form with your clinical leadership and counsel first, and adapt anything that does not match how your practice runs.
